Request Masks From Campus Safety & Security

The masks Campus Safety & Security are fabricating are prioritized for our essential employees first, but will be provided to students while stock lasts. Please be aware that you may not receive masks immediately. They will be distributed according to the priority established by the university's Emergency Operations Committee.

Questions, concerns, and reports of device-related adverse events/effects (in accordance with the United States Food and Drug Administration (FDA) Medical Device Reporting regulation) can be sent to byuhsafety@byuh.edu.

Cloth face coverings distributed by BYUH's Campus Safety office are for BYUH students, faculty, and staff. Cloth face coverings are not to be used as respiratory protection, but to assist in limiting the spread of COVID-19. Recipients are responsible for their face coverings, which should be laundered/cleaned often, and cannot be shared or traded with others. These face coverings are not for sale/resale. By filling out the following form, you indicate that you have read and understood this acknowledgment.
